I noticed a couple of bloopers recently when reading Daredevil #4. In the panel below, Daredevil says he can tell Karen is to his left, behind Killgrave the Purple Man. Nope; Karen is standing with some thugs to Daredevil's right. You could argue that Daredevil is just mistaken, but the specificity of the dialogue about her breathing makes me classify this as a writer blooper; Stan Lee must have thought Karen was out of frame and tried to account for her in the dialogue, not realizing that the back of her head was visible in the lower right of the panel.
The next error spans two pages. Daredevil is fighting Killgrave and his goons in an apartment. Killgrave grabs Karen and races out the door into the elevator in the hall. Daredevil follows and kicks Killgrave backwards into the elevator...
... and somehow they are immediately on the rooftop in the next panel. This looks like an assembly/editing error in which some intervening panels were cut for space or something. The skylight hallway in the panel above has an open door, presumably leading to the roof. See the skyscraper across the street, partly visible through the skylight and partly through the open door? The outside of that door and the skylight can be seen in the second panel below.

Reading through 1960s Daredevil, they used to give No-Prizes to people who spotted factual errors or plot holes. But at some point they changed the rules so that you had to explain apparent errors and then make up some reason it wasn't really an error.
Here's a pretty well-known one--Stan referred to the Hulk as "Bob Banner" throughout Fantastic Four #25. Previous to this appearance Dr. Banner had always been known as Bruce.
Readers wrote in about this goof, and so in #28's lettercol Stan announced that Banner's name was actually Robert Bruce Banner!
Explanation: the stress of changing (and he looks to be in mid-transformation in the bottom panel) is akin to being drunk. You get names mixed up. How many times have we been called the wrong name by a drunk at a wedding party?
